3/23/2023 EnchantaVaughn Approved (AM) Upon arrival, client arrived to school in a negative mood. Client behaviors were viewed as aggressive and careless. Client displayed impulsive behaviors due to following behind others who were being disruptive in the hallway. Client displayed defiant and disrespectful manners towards mental health professional when confronted to get to class or he would lose his break during psycho social. Client did not seem to care and shrugged his shoulders. Client was prompted 1-2 times before getting back on task. Client shifted his positive attitude into a negative attitude. Client was asked to go see his counselor. Client was welcoming to his peers as he transitioned into social setting. Client was compliant in session, due to following orders presented by QMHP and managing his disorder in a productive way. Client was observed displaying inappropriate behaviors while transitioning from social environment back to class and was reprimanded by his teacher to leave his attitude at the door or he would be given additional consequences. Client went to his seat to and stayed to himself for the rest of the morning. (PM) Client went into his lunch period with a nonchalant attitude. Client appeared to be moody and needing one on one. Client was confronted by QMHP. Client was inquired how he was feeling. Client shared negative affirmations at this time. Client have to be willing to make better choices in school in order to achieve all objectives on a consistent basis. Client has shown minimum progress with following treatment plan. Client was seen having difficulty complying to rules and demonstrating inappropriate manners in gym, lunch, and classroom settings. (EOD) Client exhibits disrespectful manners when confronted and redirected. Client was seen going into session with an aggressive tone and walking in the opposite direction while stating, "I want to go home" "I don't want to be here". Client expressed sad and serious comments about hating school and not liking his teacher. Client was encouraged to think positive and to try to give it a chance. Client was encouraged of how lucky he is to be able to go to school and to be given so much love and support. Client was educated that life will get hard, but you have to believe in yourself and be thankful because it could be a lot worst. Client is required to practice and review objects/expectations over the weekend with parent/siblings. Client partially met objectives, as witnessed by his half way listening and following directions and showing some progress in needed areas.
